Adult Carer – Self Registration

Registration as an adult carer enables you to access a range of services from Barnet Carers Centre and the London Borough of Barnet.  These services include our emergency card scheme which will provide for the person you care for in the case of an emergency, a Borough Leisure Pass, access to grants, advice and training, a discount scheme and much more.

Normally, the registration process takes place during an appointment with one of our team either in person or over the telephone.  However, we know that your role as a carer can often take up a great deal of your time and every minute can be precious.

For that reason, we have made the form available online.  If you fill it in, our team will then produce a care plan for you, email it to you and then, if needed,  go over it with you on the telephone.

If you would still like to book a face to face or telephone appointment you can do that here.  We try to make as many appointments available each week as possible, however, in times of high demand we can sometimes have up to a three week wait for a face to face appointment and a two week wait for a telephone appointment.

All information in the form below will be kept securely and will only be used to support you.  You will be asked to indicate your consent for us to use the information at the end of the form and you will not be able to submit the information without that.  We comply to all data protection legislation and regulations and your data is safe with us.  Filling in this form will save you time and allow you to access our services more quickly.  It is necessary for you to register with us using an email address.  This enables us to send you your action plan as soon as possible.
